As of February 15th, 2018 in service of our mission the Saluda Counseling Center will upgrade our treatment model for integrative care by requiring a counselor for all patients interested in medication management. Patients will be required to schedule their first appointment with a counselor and keep two additional appointments with a counselor before we schedule an appointment with a medical provider. At a minimum patients will be required to follow up with their counselor every 3 – 6 months as determined by the counselor if the patient plans to see a medical provider for medications.
If you are currently being treated by an outside medical provider who is referring you to us, please ask them to send the referral information ahead of your first appointment.
Clients requesting an appointment for counseling only will not be required to receive medical services.

Preparing For Your First Appointment
The Saluda Counseling Center is here to serve your needs, and we want your first appointment to be easy to plan for. Before your first appointment you can download the forms you will need to complete when you arrive, just click on the links below and print the documents that you will need.
We ask that you arrive 20-25 minutes early for your appointment in order to ensure all necessary forms are completed. Please bring your completed paperwork to the first appointment so that when you arrive we can make sure your first check in is quick and easy.
In addition to bringing your new patient forms please remember to bring the following items to your first appointment:

- Picture ID
- Insurance Card(s)
- Copay or other amount due for your visit
- List of current medications
- Other important forms when applicable, such as: Power of Attorney forms (Medical or other), or Custody paperwork
- Medical records from your previous provider (if applicable, when available)
- Documents related to EAP (Employee Assistance Program) visit authorizations (if applicable)
